What is Hyperstat?
Hyperstat given orally raises blood sugar by slowing the release of insulin from the pancreas.
Hyperstat is used to treat low blood sugar (hypoglycemia) related to certain cancers that affect the pancreas and cause it to release too much insulin.
Hyperstat may also be used for purposes other than those listed in this medication guide.
Hyperstat indications
Hyperstat® (ORAL Hyperstat) is useful in the management of hypoglycemia due to hyperinsulinism associated with the following conditions:
- Adults: Inoperable islet cell adenoma or carcinoma, or extrapancreatic malignancy.
- Infants and Children: Leucine sensitivity, islet cell hyperplasia, nesidioblastosis, extrapancreatic malignancy, islet cell adenoma, or adenomatosis. Hyperstat® may be used preoperatively as a temporary measure, and postoperatively, if hypoglycemia persists.
Hyperstat® should be used only after a diagnosis of hypoglycemia due to one of the above conditions has been definitely established. When other specific medical therapy or surgical management either has been unsuccessful or is not feasible, treatment with Hyperstat® should be considered.
How should I use Hyperstat?
Use Hyperstat suspension as directed by your doctor. Check the label on the medicine for exact dosing instructions.
- Shake well before using.
- Use a measuring device marked for medicine dosing. Ask your pharmacist for help if you are unsure of how to measure your dose.
- Hyperstat suspension works best if it is taken on a regular schedule.
- Continue to use Hyperstat suspension even if you feel well. Do not miss any doses.
- If you miss a dose of Hyperstat suspension, take it as soon as possible. If it is almost time for your next dose, skip the missed dose and go back to your regular dosing schedule. Do not take 2 doses at once.

Use: Labeled Indications
Hyperinsulinemic hypoglycemia: Management of hypoglycemia due to hyperinsulinism due to the following conditions in adults (ie, inoperable islet cell adenoma or carcinoma, or extrapancreatic malignancy) and infants and children (ie, leucine sensitivity, islet cell hyperplasia, nesidioblastosis, extrapancreatic malignancy, islet cell adenoma, or adenomatosis; may be used preoperatively as a temporary measure, and postoperatively, if hypoglycemia persists).
Note: Consider treatment with Hyperstat when other specific medical therapy or surgical management for hypoglycemia due to the above conditions either has been unsuccessful or is not feasible.
Hyperstat description
A benzothiadiazine derivative that is a peripheral vasodilator used for hypertensive emergencies. It lacks diuretic effect, apparently because it lacks a sulfonamide group. [PubChem]

Patients should be under close clinical observation when treatment with Hyperstat® is initiated. The clinical response and blood glucose level should be carefully monitored until the patient's condition has stabilized satisfactory; in most instances, this may be accomplished in several days. If administration of Hyperstat® is not effective after two or three weeks, the drug should be discontinued.
The dosage of Hyperstat® must be individualized based on the severity of the hypoglycemic condition and the blood glucose level and clinical response of the patient. The dosage should be adjusted until the desired clinical and laboratory effects are produced with the least amount of the drug. Special care should be taken to assure accuracy of dosage in infants and young children.
Adults and children:
The usual daily dosage is 3 to 8 mg/kg, divided into two or three equal doses every 8 or 12 hours. In certain instances, patients with refractory hypoglycemia may require higher dosages. Ordinarily, an appropriate starting dosage is 3 mg/kg/day, divided into three equal doses every 8 hours. Thus an average adult would receive a starting dosage of approximately 200 mg daily.
Infants and newborns:
The usual daily dosage is 8 to 15 mg/kg divided into two or three equal doses every 8 to 12 hours. An appropriate starting dosage is 10 mg/kg/day, divided into three equal doses every 8 hours.

What other drugs will affect Hyperstat?
Since Hyperstat is highly bound to serum proteins, it may displace other substances which are also bound to protein, such as bilirubin or coumarin and its derivatives, resulting in higher blood levels of these substances. Concomitant administration of oral Hyperstat and diphenylhydantoin may result in a loss of seizure control. These potential interactions must be considered when administering Hyperstat® Capsules or Suspension.
The concomitant administration of thiazides or other commonly used diuretics may potentiate the hyperglycemic and hyperuricemic effects of Hyperstat.
Drug/Laboratory Test Interactions
The hyperglycemic and hyperuricemic effects of Hyperstat preclude proper assessment of these metabolic states. Increased renin secretion, IgG concentrations and decreased cortisol secretions have also been noted. Hyperstat inhibits glucagon-stimulated insulin release and causes a false-negative insulin response to glucagon.

As well as its needed effects, Hyperstat (the active ingredient contained in Hyperstat) may cause unwanted side effects that require medical attention.
Stop taking Hyperstat and get emergency help immediately if any of the following effects occur:
Rare
- Chest pain caused by exercise or activity
- confusion
- numbness of the hands
- shortness of breath (unexplained)
Severity: Moderate
If any of the following side effects occur while taking Hyperstat, check with your doctor or nurse as soon as possible:
More common:
- Decreased urination
- swelling of feet or lower legs
- weight gain (rapid)
- Fast heartbeat
- Fever
- skin rash
- stiffness of arms or legs
- trembling and shaking of hands and fingers
- unusual bleeding or bruising
Minor Side Effects
Some Hyperstat side effects may not need any medical attention. As your body gets used to the medicine these side effects may disappear. Your health care professional may be able to help you prevent or reduce these side effects, but do check with them if any of the following side effects continue, or if you are concerned about them:
Less common:
- Changes in ability to taste
- constipation
- increased hair growth on forehead, back, arms, and legs
- loss of appetite
- nausea and vomiting
- stomach pain
This medicine may cause a temporary increase in hair growth in some people when it is used for a long time. After treatment with Hyperstat has ended, normal hair growth should return.

What is the most important information I should know about Hyperstat?
You should not take this medication if you are allergic to Hyperstat or diuretics (water pills).
Oral Hyperstat should not be used to treat occasional hypoglycemia related to diet.
Before taking Hyperstat, tell your doctor if you have congestive heart failure, kidney disease, gout, high cholesterol, or low potassium.
Take this medication exactly as prescribed by your doctor. Do not take it in larger amounts or for longer than recommended. Your doctor may occasionally change your dose to make sure you get the best results from this medication.
If your blood sugar gets too high (hyperglycemia), you may have symptoms such as increased thirst, loss of appetite, fruity breath odor, increased urination, drowsiness, dry skin, nausea, and vomiting. Tell your doctor right away if you have any of these symptoms.
To be sure this medication is helping your condition and not causing harmful effects, your urine will need to be tested often for the presence of glucose (sugar) or ketones. You may be able to do this testing at home. Call your doctor if you have any abnormal test result.
Your doctor may also want you to have blood or urine tests at regular intervals. Do not miss any scheduled appointments.
Follow your doctor's instructions about any restrictions on food, beverages, or activity while you are taking Hyperstat.
If your condition does not improve after taking Hyperstat for 2 to 3 weeks, stop taking the medication and talk to your doctor.
